To participate in our distribution event, families must complete the form below. Once your form is submitted, a team member will review your application and send an advisory notice to the primary email address listed.

Applicants must be residents of Milford, CT, provide proof of residency for all members listed on the application, and have children 11 years of age or younger.

2 Proofs of Residency are required:

  • Adults: A copy of a government-issued photo ID, driver’s license, state ID, ETC
  • Children of school age: A recent report card is required. A screenshot from the Milford Power-School portal is acceptable. Screenshot example here.
  • Non-school-age children: A copy of a utility bill, lease agreement, or a letter from your pediatrician verifying residency.
